A 25-year-old woman was cannulated for extracorporeal membrane oxygenation (ECMO) after arrest from eosinophilic myocarditis. A 21-F venous HLS cannula (Maquet Getinge, Gothenburg, Sweden) was inserted via the right femoral vein in the intensive care unit with a left femoral artery return cannula, without issues noted at insertion. Subsequent transthoracic echocardiogram suggested that the cannula tip was not within the right atrium but was present more inferiorly.
