The authors describe an anatomic variant of left adrenal venous drainage in a 50-year-old woman with primary hyperaldosteronism who was referred for adrenal vein sampling (AVS). The patient underwent unstimulated sequential AVS via a right femoral venous approach and was found to have dual left adrenal venous drainage.
